The National 4H Conference Center host requires that parents of minors sign this page.
We appreciate their concern for safety. LTIA staff take student safety very serious and review safety precautions with students everyday when we are in DC.
The conference center is providing us with rooms that accommodate four people to a room. An LTIA staff member is assigned to stay in each room with three students.
There are several ways to send us the signed form.
1. You can print and mail the signed form to:

2. Your child can bring the signed form to the LTIA conference and give it to LTIA staff when he/she arrive.
3. Sign the form digitally, and send it by email.
a. Open the file, click on ‘Sign’ in the upper right hand corner of the open page.
b. Select ‘Add Text’ to fill in the blank lines on the authorization form.
c. Select ‘Place Signature’ to add Signature. It needs to be the signature image and not typed text.
d. Then click on ‘File’ and select ‘Send file.’
